COVID-19 Policies and Procedures:

If you will be coming in for an appointment soon, please take a moment to read through our COVID-19 policies, and do not hesitate to contact us with any questions.

• If you or anyone in your household has been in contact with a person that has been diagnosed with COVID-19 in the 14 days prior to your appointment, kindly notify us before your appointment. If you are experiencing symptoms of COVID-19 such as fever, shortness of breath, persistent cough, or stuffy/runny nose please let us know so that we can reschedule your appointment when you are feeling better.

• You may expedite your check-in by completing online COVID-19 screening here. Please update the staff if your responses need to be updated at your appointment time.

• We encourage unvaccinated individuals to wear a face mask at all times during their visit.

• Please take a moment to wash your hands upon entering our office. Hand sanitizer is also readily available.
